An ACO’s total savings are determined by subtracting its total beneficiary expenditures from total expenditures. The resulting value represents savings (or, in some cases, losses) shared between the ACO and the Centers for Medicare and Medicaid (CMS).

Which ACOs in California generated the greatest savings in 2024?

Our data reveals that Dignity Health Care Network LLC led all California ACOs in 2024, generating over $28 million in gross Medicare savings. Based in San Francisco, this ACO serves more than 68,000 patients, making it one of the largest organizations in the state.

Following closely behind, University HealthCare Alliance Accountable Care Organization achieved savings of more than $27 million in 2024. With a patient population of over 23,000, this ACO, located in Newark, demonstrates the potential for medium-sized ACOs to drive significant cost reductions. Through its focus on patient-centered care and collaboration across providers, this organization has consistently ranked among the top performers in the state.

In third place is Family Choice ACO LLC, which generated more than $14 million in savings while managing the care of over 11,000 patients in Westminster. Despite its smaller size compared to other top-performing ACOs, Family Choice’s success highlights the impact of efficient coordination, communication, and shared goals in delivering high-quality healthcare.

The top ACOs in California had an average gross savings of $13.4 million, with a patient population averaging 19,446. This results in approximately $690 in savings per patient.
